Home Visits

Home visits are offered to Medicare patients only. Home visits are determined after an evaluation with the healthcare team.
Here at PedIM Healthcare we believe very strongly in the triple AIM Model.

• Improving the patient experience of care (including quality and satisfaction)
• Improving the health of populations
• Reducing the per capita cost of health care

We understand the challenges that some of our patients face due to having significant difficulty when traveling or being unable to travel for needed medical care. Some conditions include:

• Severe mobility impairments
• Terminal illnesses
• When travel is contraindicated due to the person's chronic condition
• Where transportation to medical appointments is limited due to geographical considerations
